FMLA and disability paperwork often require detailed review and completion by our medical team. To ensure forms are completed accurately and in a timely manner while allowing our providers to continue delivering quality patient care, there is a $25 processing fee for all FMLA and disability paperwork requests.

Once payment is received, please allow up to 7 business days for completion. To help avoid delays, please ensure all required information is submitted and that your name is clearly listed on all paperwork provided to our office.

Once completed, our office will fax or email the forms directly to the appropriate destination. A copy can also be emailed to you upon request.
